Life as a medical student #19 My Shaking Hands: Suturing for the first time

Displaying 2016-07-29 12.22.45 1.jpg

The operating theatre with its bright lights, sharp steel and demanding scrub nurses is a land of mystery for most medical students. Having seen so many operations done (from far corner of the room, of course), I had never stitched up an actual person. Ironically, my first chance of sewing up an incision happened when I was at Tzu Chi Hospital in Taiwan. It was a reconstructive surgery after resection of retromolar trigone cancer. Doctor Kang and Doctor Weng had removed the anterolateral thigh (ALT) flap and was now beginning to re-connect blood vessels at the neck.

''Why don't you sew up the site on the leg where we harvested the flap?'' Doctor Weng said.

I was a bit startled, but I nodded. Looking down at the patient's leg, there was a 4-inch incision to sew up. It wasn't too deep. Probably just need a single layer of simple interrupted suture. Very quickly the nurse handed me a forceps, a needle-holder and a length of 3.0 nylon. I tried to make myself comfortable with the instruments like how I used to practice in classes, and concentrate on the incision in front of me. The first few stitches took me an eternity to put in - using the forceps my left hand grasped and everted the skin edge slightly, my right hand pronated and supinated to allow the needle to pierce the skin - I don't know why but that particular moment made my hands tremble a little. While taking a deep breath in, I imitated how the surgeons normally wrap the suture around the needle holder to form the loop, and then tie the knot. And finally cut the suture leaving a 5mm tail. That's my first knot! I looked up and exchanged glances with Doctor Weng. He seemed satisfied. With Doctor Weng continue guiding me, I began suturing with more confidence.

Slowly, but more and more smoothly.

Skin pads, banana skin, orange peels? Nothing beats suturing a live patient. I can now suture better. Really.

Life as a medical student #17 Be A Different Doctor

When I was in Taiwan, I went to Shifen train station to release a sky lantern, inspired by the movie 《那些年,我们一起追的女孩》(You are the apple of my eyes). I first prayed for health for my family and friends. Then without hesitation, next on my list was: I want to be a different doctor.

Can you imagine that? In less than two years, I am going to be a doctor. I want to know, what kind of doctor am I going to be? I have seen both the incredibly passionate ways some of the doctors in the hospital treat their patients, and the behaviours of the others that I will never emulate in my own practice in the future. I've seen medicine serve some of its patients well and completely change their lives, while failing others. I had the chance to work on extremely effective teams and surprisingly dysfunctional ones, too. I've shared intimate moments with patients who placed their confidence in me - rather than the other senior doctors of the team - to treat them well.

Therefore I know, without a doubt, what kind of doctor I want to be.

But the truth is, I am scared. I don't know what future lies. I am afraid that one day, after committing my life to this profession, the sparks won't be there.

What if, one day I gradually become the kind of doctor I hated the most?

I believe most doctors went into medicine to help people. We want to practice medicine the right way, but too many forces today are causing us to have diminished enthusiasm for medicine. While taking care of people in their most vulnerable states, we make ourselves somewhat vulnerable in the process, too. It is hard, to stay motivated.

Therefore, I write. Ten years from now I am going to read what I have written today as a student. I want to replay the image of that female doctor, in the small consultation room of HKL, who would bend down on her knees just to adjust the footrest of the wheelchair for an elderly patient. Or the one who would put his pen down just to listen to what the anxious-looking old man was trying to tell him, and acknowledged the worry in his eyes.

Don't you ever forget that.

[TRAVEL & FOOD] The Best Food in Taiwan - Time to eat! And appreciate

These days, thin is in. ''Food'' is a four-letter word. Almost everyone is on a diet. Me too, on my way achieving my desired body shape. I mean, look at the fats on my belly and thighs. *crying at a corner*  But wait, did you just say food in Taiwan?

There is no love sincerer than the love of food.
-George Bernard Shaw (1856 – 1950)

One of the best things about Taiwan is undeniably the food. It is simply irresistible! Mention Taiwan and anyone would immediately think of night markets and street food. In Taipei alone, there are many night markets that offer an amazing array of mouth-watering street food. Here I have compiled a list of must-try food in Taiwan, and hey, each of them has a secret message behind it! TIME TO EAT! And appreciate :)

#1 Braised pork rice (滷肉飯)

image

"Where there's a wisp of smoke from the kitchen chimney, there will be braised pork rice," goes the Taiwanese saying. In days gone by when meat products were unaffordable for most regular families, this humble dish of minced pork braised in soy sauce, rice wine, and five-spice over a bed of rice became a cost-effective but mouth-watering meal for the whole family. This explains why it holds a special place in the hearts of the Taiwanese. A little sweet, a little salty, simple but addictive.

Appreciate the simple things in life

#2 Fried Stinky Tofu (炸臭豆腐)

image

Just as the name suggests, this tofu is extra stinky - the odour can be smelled from 20 meters away! The smell of it comes from it being preserved and fermented from beancurd. People who love it say that the stronger the smell, the better the taste. Just like durian, you either love it or hate it. Silky soft inside and crunchy on the outside. Well you've got to try it to know!

Never judge a book by its cover

#3 Oyster Vermicelli (蚵仔麵線)

image

Another must try Taiwanese dish - a bowl of hearty oyster vermicelli that can be enjoyed at any time of the day. A bowl of good oyster vermicelli should have a thick, flavorful soup base while the thin rice noodles and oysters retain their distinct texture. Some people add chopped intestines for a funky dimension to the soup. Gooey and slurpable.

Warm and Homey, Rich and Hearty

#4 Fried Chicken Cutlet (炸雞排)

image

Many street food dishes have relatively petite portions, but it is definitely not the case with Taiwan's fried chicken cutlet - they want it big - and the bigger, the better. Just look at the massive size of this meat! It is moist yet tender. Fried to perfection. Don't miss the one from Hot Star (豪大大雞排) at Shilin Night Market in Taipei. Always a long line in front of the stall, but definitely worth the wait.

In order to be irreplaceable one must always be different

#5 Oyster Omelette (蚵仔煎)

image

If you like oysters or seafood, this is a great choice for you. If you like omelettes too, then you've hit the jackpot! More commonly known amongst Taiwanese as 蚵仔煎 pronounced ''oh-ah-tzen'', it is an amazing yet perfect combination of yummy oysters embedded in a chewy omelette. What makes it even better is the sweet and sour sauce smeared on top. Yummm.

A perfect combination can do wonder

#6 Small Sausage in Big Sausage 大腸包小腸

b2ap3_thumbnail_10602742_10204044321410486_2136220716_n.jpg

Directly translating to ''Small Sausage in Big Sausage'', this was a creation of Taiwanese street stall vendors. It's like a hot dog, but instead of a bun, sticky rice is stuffed inside sausage casing to make an oversized rice sausage. That's then slit open to stuff in a pork sausage. As easy as that, but good enough to make my mouth waters.

Simplicity at its best

# 7 Crushed ice dessert (剉冰)

The best dessert of all time. Especially when the weather is so hot and humid in Taiwan, try mango shaved ice! The incredibly fresh cuts of mango is not even the best part. The shaved ice texture is mind-blowing! It is like creamy, sweet, and a cotton-candy like texture. Imagine sharing this bowl of shaved ice with your friends in a hot afternoon. Pure heaven.

The best things in life are meant to be shared

#8 Pearl Milk Tea (珍珠奶茶)

A trip to Taiwan wouldn't be complete without a sip of its most iconic drink. Pearl milk tea (or 'bubble tea' as some call it) is a refreshing mixture of black tea, milk, and chewy tapioca balls. Especially refreshing on a hot summer day. You would argue that Malaysia has Chatime and Gong Cha too, but to taste the authentic flavour of pearl milk tea, Taiwan it is.

Happiness is a cup of bubble tea under scorching hot sun

I believe writing this post doesn't do justice for how delicious the food are - I really wish to try them all!  I have heard so much about Taiwan from my friends but I've never been there. So when I know that The Butterfly Project & Taiwan Tourism Bureau Kuala Lumpur are going to send a lucky butterfly and her friend to Taiwan, I thought I would just try my luck! :) The trip includes a 5D4N stay, return air flights, accommodation and tour worth RM8,000. If you are a blogger too, come and join by telling them why you want to go Taiwan and how you will enjoy your trip inspired by 1 of the 3 themes available: Time to Eat (for foodies) / Time to Shop (shopaholics!) / Time for 2 Wheels (LOHAS lifestyle). I can't wait for summer, I am expecting a scrumptious food tour in Taiwan already :D

[BEAUTY REVIEW] Ludeya V Line Chin Mask & Yufit Bodyboom

For too many women, being "model thin" is a very real aspiration. It's no wonder. Bombarded with countless media images of thin female models and actresses who look beautiful and appear happy, many women are unhappy about their own bodies. If you are one of them, iQueen has a solution for you with the two products selling on their website!

P/S: Read my previous review on iQueen here, here and here!

Ludeya V Line Chin Mask

I fell in love with Ludeya masks ever since my last review about it. (You can read about it HERE!) So when I received another Ludeya mask from iQueen, I was really excited.

This time, Ludeya wants you to say goodbye to your double chin, chubby cheeks and baby fat! For women, the ideal and most sought after face type is the ever-so-popular V-Line shape. While many celebrities go under the knife for the V line, Ludeya V Line Chin Mask is certainly an alternative for you! It has pulling, firming and polarizing function to make you look photogenic in just 15 minutes.

There are 4 V Line Chin Masks in a box.

How to use?

To use, hang the up lift hook of the chin mask on the ears as shown in the picture below. Press gently with fingers to ensure it fits to skin. Let the mask sit for 20-30 minutes.

Let's see the results!

I have very chubby face to begin with, so the mask was made just for me. I could feel a slight lifting effect after just one use, more obvious on the right side of my face. The mask has a very pleasant smell just like the other Ludeya masks. It is elastic, and it doesn't cause any discomfort. Another reason for me to love Ludeya products more. :D

Goodbye double chin!

If you are interested to purchase, you can get it from iQueen HERE at only RM14.45 (15% discount), grab it while stock lasts!

Yufit Body Boom Anti-Cellulite Slimming Mousse

The second product that I am going to introduce is Yufit Body Boom Anti-Cellulite Slimming Mousse, the great body fat killer! It is a clinically tested foam originated from Korea that is formulated using the most revolutionary cosmetic technology that helps to reduce the appearance of cellulite in the buttocks, arms, thighs, calves and abdomen areas. It also helps to improve elasticity of the skin.

There are two types of Body Boom - the hot and the cold type. Basically, they function the same.

In this review, I will just try the hot one! The 200ml package has an average duration of 20 days.

How to use?

Shake the bottle, turn it upside down and dispense a small amount of fisioline on the area concerned. Smooth the product in using soft circular movements from from the peripheral areas to the centre for complete absorption. Repeat this procedure twice a day as an initial treatment. Do take note that the product absorbed quickly, so it is not recommended to apply the product more than once on the same area. And also do not use before going to bed.

When it was first applied, it was fresh and smooth just like mousse. However, what happened after that amazed me! The product seems to ''crackle'', I could feel a sort of tingling, light bubbling, followed by a burning sensation on the skin which lasted for around 5 minutes. For a moment I thought my skin was on fire!

After a while, the crackling stopped. Massage it well over the area to get maximum benefits. As you can notice from the picture below, there was immediately a slight reddening on my skin, proving that the microcirculation has been reactivated. The crackle technology has the capacity to effectively enhance the activity of the active ingredients in the epidermis within the very first minutes of application.

I used the product mainly on my thigh and stomach, twice a day for one week. Surprisingly, my skin appears to be more tonic and compact, which I couldn't even achieve when I work out at the gym! You should try it too! You can get it from iQueen HERE (heating) and HERE (cooling) for only RM69.00.

[BEAUTY REVIEW] Sexylook Black Mask & Blackhead Set (Coffee)

Suppose you need to attend an official function or a party the very next day. Suddenly, you remember those awful-looking blackheads on your face. Though, they are hardly noticed from a distance, but they look bad if seen from close. What will you do? Well, I have a solution to make blackheads go away instantly! Introducing.... *drumrolls*

Sexylook Coffee Bean Black Head Pore Cleanser Set

Yes you didn't hear me wrong. Coffee beans! It is the new upgrade version of the previous highly raved Sexylook Strawberry Black Head Pore Cleanser Set. This product is very famous and it's a big love in Taiwan! It was once featured in 女人我最大, a very famous Taiwan beauty programme. Even Xia Xue, the top celebrity blogger once recommended this product. So you know it is a must to try! This time, it uses Arabica coffee bean extract as main ingredients to help skin renew and exfoliate. Remove blackheads thoroughly with only three simple steps! Unbelievable isn't it? Coffee with beauty benefits! Let's test and see how effective it is.

The set of three consists of a sebum softener, a blackhead remover and a shrink toner. They are nicely labeled with 1, 2 and 3 so that you won't go wrong with the steps. However, the description behind the box is written in Chinese as it is a product from Taiwan. Not very user friendly, therefore I will just briefly explain the steps in English here. :)

How to use?

STEP ONE Soften

After face washing, apply some sebum softener on the areas you wish to focus on and massage. This step is important because it softens your blackheads and sebum, making it easier to remove later on. The solution is very watery so be careful when you are pouring it! When you are done, rinse with water. Remember, it is very important to rinse your face after you apply the sebum, as the sebum is slightly oily and this layer of sebum will actually prevent the mask from sticking on the blackheads later on.

STEP TWO Remove

Apply a thick layer of black heads remover or the so called mask on areas which are rich with stubborn blackheads. Leave it until it dry. It takes about 20-30 minutes to dry up completely. And wow, I really smell coffee!

When it's time, gently peel it off, starting from the bottom to top, in an upward direction. The mask that is unsuccessfully peeled off can be directly washed with water. And tadaaa! Byebye blackheads! Can you see some whitish stuffs standing up? This sounds a bit gross but these are the blackheads, whiteheads, dirt or whatever you celled it. Yucks, I hate you blackheads. It may not be able to remove 100% of my blackheads at once, but it sure helps a lot!

STEP THREE Shrink

Lastly, don't forget to apply appropriate amount of shrink toner on the treated skin to leave the skin soothing and pore refining. It closes up our pores so that the dirt will not get in easily. For best results, use this blackhead cleanser set once or twice a week. :)

If you are interested to try, grab yours from iQueen here: http://www.iqueen.my/en/blackhead/237-sexylook-coffee-bean-black-head-pore-cleanser-set.html :D

Sexylook Superior Black Mask (With Neck)

Another product that I am going to introduce today - celebrities & bloggers favorite - Sexylook Superior Black Mask series! So blessed to get to try so many masks recently, if you haven't read my previous post about Ludeya Bio Cellulose mask, you can read it here because I think that's a good mask which works on me too. Back to the mask today, what's so special about this mask? You probably already guessed it by it's name - yes they are BLACK! It uses advanced technology to infuse activated carbon into the pure pulp cotton. Derived from natural ingredients. :) It also claims to have one whitening laser effect just after three times using this mask.

There are five individually wrapped mask sheets in one box. However, iQueen is so kind that they gave me a total of six masks, with three different functions! Hehe thank you iQueen. :) Another special thing about this mask is that it has the extended area for our necks as well.

Again, the description are written in Chinese, with some Japanese this time. Despite the cute anime girl and the Japanese characters on the kawaii packaging, Sexylook is in fact a 100% Taiwanese sheet mask brand. Only that it is made of 100% pure cotton imported from Japan. :)

Choose your favourite! For review purpose, I chose the golden one to try.

Blue: HYDRATING
Pink: BRIGHTENING
Golden: MOISTURIZING

One thing I've noticed about this black mask is that there is no plastic layer between the folding like usual mask. I could see some golden bits on the black mask, and I was told that they are 24K gold bits :O

The pure cotton sheet mask was so soft to touch and was extremely comfortable to wear. It is the softest sheet mask I have ever used! The texture of the mask was perfect, not too thick and not too thin like the transparent silk sheet masks. The scent of the serum was mild and pleasant.

After cleansing and toning, apply mask on the face for 10-15 minutes then remove it. If there is remaining essence on the face, gently massage it with fingertips to help absorption. I love that the mask fits me perfectly, without any bubbles of air after I applied it on my face. Thumbs up for it! There was residue serum inside the package so I used them on my hands and legs.

So, did the mask deliver what it promised? I will certainly have to try this mask out for a longer period of time before confirming its claims, but after one trial, I quite like it as it really makes my skin look appealing and better than before.

Want to get your own black mask? Good news, you can get it now at iQueen with 20% discount! What a steal. Limited period only. Grab now at: http://www.iqueen.my/en/119-promo5.html :D

iQueen is like my one stop beauty centre nowadays. So in love with all their products!

[BEAUTY REVIEW] Ludeya Bio Cellulose Mask & Simply Barley and Red Bean Water

Hello everyone! Today I am going to share with you the two products that I received from iQueen - Ludeya Bio Cellulose Mask & Simply Barley and Red Bean Water.

LUDEYA Bio Cellulose Mask

Ever wondered what is S.H.E Selina Jen's beauty secret? Despite her burn accident in 2010, Selina is able to maintain pretty, smooth and rejuvenated skin. All because of LUDEYA Bio Cellulose Mask!

Ludeya, established in year 2003, high efficacy yet authentically dermatologist beauty solutions masterminded by the elite team from Korea, Japan, Europe & USA, products are mainly made in Korea & Taiwan. Ludeya insists Non Artificial Colourings, Non Artificial Fragrance & Preservatives, hypo allergenic products that suits all skin type. Bio Cellulose, also known as ''artificial skin'', was medically used as a substitute for skin to treat various types of wounds and burn injuries. This allows Bio Cellulose Mask to completely bond with skin to deliver the exclusive SEANOL® essence deep into the dermis, and effectively reduce skin sensitivity. The Algae Polyphenol Extract was well-known for its skin repairing, brightening and hydrating benefits. It provides intense hydration to the skin, leaving it moisturized, radiant and more youthful looking.

Selina has been using LUDEYA Bio Cellulose Mask for 2 years. Then only she agreed to be the ambassador of LUDEYA. LUDEYA helps her to recover her face skin after the burning incident, even beauty guru Kevin has been using Ludeya as his all times favourite, so you know it's really effective!

Pink: PORE REFINING + MOISTURIZING
Silver: WHITENING + MOISTURIZING
Black: REPAIRING + MOISTURIZING

How to use?

STEP 1
After thorough cleansing, remove the mask from the pouch.

STEP 2
Remove the pearl paper and
apply the mask on face and fix it with fingertips using gentle pressure,
then remove the blue layer.

STEP 3
Ensure the mask adheres well to the skin with no bubble of air in between.

STEP 4
Relax and let it acts for 15-20 minutes.

STEP 5
Remove the mask and gently massage remaining essence into skin.

Overall, the quality on the masks, all the way from the packaging to the product itself, is fantastic! I tried the silver one, which has the function of deep whitening, hydrating, brightening and anti dullness. I did not experience any irritation, nor did I see extremely pronounced results. Nevertheless, my skin definitely looked less dull than it did prior to use, which is largely due to the boost of hydration. The best part is, my face feels supple and bouncy!

Interestingly, considering how drenched the mask was in the product when I first removed it from the packaging, the amount of moisture left on it when it was removed after the half hour mark has pronouncedly diminished which indicates that the formula on these is very effective at penetrating the skin. The gel-like, rubbery mask adhered on my face perfectly without any trouble of slipping off or adjusting it over and over again. It is something really new for me to try because I have never put on a mask that comes in three layers. The one that I usually put on is just two layers by itself.
